The purposes of the Self-Assessment Guidelines are to:
1) Establish a consistent set of minimum expectations for each level of perinatal services.
2) Enable each institution to provide the consumer with a consistent level and quality of perinatal services.
3) Recognize the capabilities, commitment and resources of institutions that are beyond the minimum expectation for their level of perinatal services.
With this perspective in mind, the Colorado Perinatal Care Council, as a non-regulatory body representing providers and consumers of perinatal services and the interested public at large, adopts a revised set of guidelines for participating institutions in Colorado. All hospitals that provide perinatal care are asked to participate in this self-assessment survey.
The following list includes all of the necessary documents you will need in order to complete the self-assessment process. The application, instructions, guidelines and appendices are all located on different tabs within the specific spreadsheets listed below.
